During your first visit, you will be asked to fill out a Client Registration Form, and an Animal Registration Form for each of your pets. Please fill out the New Client Registration Form completely. The more information we have, including multiple phone numbers, the easier we can reach you in case of emergency or need. Please also fill out the New Animal Registration Form completely. The more we know about your pets, the better we can take care of them.

Please bring all past available records for your pets. We may have questions to ask the other veterinarian. Sometimes, the previous doctor ordered tests that we can view, thereby eliminating the need to repeat them.
